Registrations are by appointment only!!!
Please call To Make An Appointment

Please bring the following to your appointment: 

  • Student’s original birth certificate
  • Student’s social security card or number
  • Complete immunization record. Students MUST meet current VA immunization requirements prior to entry into middle school.)
  • Last report card
  • Proof of residency (examples: a lease, deed, or contract)
  • Two (2) Utility Bills
  • Two (2) forms of ID for the parent enrolling the student***
  • Name, address, phone number, and fax number of last school student attended
  • Special program information:
    • IEP if child is in Special Education
    • Gifted verification
    • ESOL (English for Speakers of Other Languages)
    • Any other scheduling recommendations
  • Custody papers (if applicable)***

The parent/guardian will need to complete the student withdrawal process inperson and sign the Prince William County Public Schools Student Withdrawal Form.
